It's my choice to receive massage therapy. I'm aware of the benefits and risks of massage. I understand that there is no implied or stated guarantee of the success or effectiveness of individual techniques or a series of appointments. I acknowledge that massage therapy is not a substitute for medical care, medical examination, or diagnosis. I've listed all medical conditions of which I am aware and will inform my practitioner of any changes in my health status.

I understand that when requesting treatment, my personal health information will be collected. I know that all information I provide will be kept confidential unless required by law. I understand and consent to the sharing of my medical information by the various care providers involved in my care and treatment.

Massage and bodywork are in no way substitutes for examination, diagnosis, or treatment by a physician. The individual providing massage and bodywork is not qualified to diagnose, prescribe, or treat any physical or mental illness.
